 Course Requirements

All students must show documentation that they are eligible to take a driver education course. The basic requirement usually includes: Showing proof of Jr. High School or High School report card which lists 8 passing grades within the last 2 semesters (within the last year). Example: Any student could pass 3 classes during their first semester and 5 classes the second semester. Students will be required to show verification by the end of the second week of class.  

 

Quick note: 
 
All students must turn in a "Blue Form" signed by the student's high school or junior high school:

  • If a student has not completed 2 high school semesters.
  • If a student is in 8th grade or is a freshmen.
  • If the high school or junior high school uses a "quarter grading system"
  • If the high school or junior high school uses "trimesters"
  • If the high school or junior high school uses "block scheduling"
  • If the high school or juinor high school does not use "semesters"

The blue form is given out by the classroom instructor on the first day of class.  You can obtain a blue form by clicking on the following link - Blue Form.
 

 

Northwest Suburban Driving School will provide information or a form to any student if they:
 

 

  • Cannot get copy of their High School report card
  • Did not pass 8 classes within the last two semesters
  • Are Home Schooled
  • Dropped out of High School
  • Enrolled in a G.E.D. Program
  • Already received a high school diploma
